Is sorbitol safe?
- WHO/JECFA: acceptable daily intake "not specified" — the highest category, meaning no number was thought necessary
- EU: permitted; foods with more than 10% added polyols must state that excessive consumption may produce laxative effects. EFSA's re-evaluation of sorbitol is still open
- US: generally recognised as safe; US law requires the words "Excess consumption may have a laxative effect" where foreseeable intake could reach 50 g a day
- LabelCop: medium-risk flag — not for toxicity, but because the gut effects are real enough that three regulators legislated a warning
What is sorbitol?
- A sugar alcohol (polyol) — about 60% as sweet as sugar, with roughly a third fewer calories
- Occurs naturally in apples, pears, prunes, peaches and other stone fruit — the reason prunes have their reputation
- The additive version is manufactured: glucose from starch (usually corn or wheat) is hydrogenated into sorbitol
- Only partly absorbed. What the small intestine doesn't take up travels on to the colon, where bacteria ferment it — that produces gas and draws water into the bowel
- It is not only a sweetener. It is also a humectant, bulking agent, stabiliser and texturiser, so it turns up in products that never claim to be sugar-free
- Label names: sorbitol, sorbitol syrup, E420, E420(i), E420(ii), INS 420 — or buried in the "polyols" line of the nutrition panel
Which foods contain it?
- Sugar-free chewing gum, mints and boiled sweets — the biggest single use
- Sugar-free and diabetic-friendly biscuits, chocolates and confectionery
- No-added-sugar jams, spreads and fruit fillings
- Protein bars, diet bars and "keto" ranges
- Soft-baked goods where it keeps texture moist — as a humectant, not a sweetener
- Some dried fruit, and toothpaste and mouthwash (not food, but a real exposure route)
- In India: very common in sugar-free mithai and diabetic-friendly sweets, mouth fresheners, supari-style products, digestive tablets and imported sugar-free gum — look for INS 420 in the ingredient list, because "sugar free" on the front tells you nothing about which sweetener was used or how much

Why do regulators disagree?
- On toxicity, they don't. Nobody has banned or restricted sorbitol. Every major regulator permits it, and JECFA gives it the top rating
- On the label, they very much do. The EU and India trigger a warning at more than 10% added polyols in the product; the US triggers it at a foreseeable 50 g a day — two completely different tests, so the same product can carry a warning in Delhi or Dublin and none in Dallas
- India writes the strictest warning. Where the EU statement is one clause about laxative effects, Indian labelling goes further and names children and post-menopausal women
- "No limit" is doing a lot of work. ADI "not specified" means the toxicologists saw no reason for a number. It does not mean the dose doesn't matter — the warning labels exist precisely because it does
- The EU file is still open. EFSA called for sorbitol data in 2023 and has not published its re-evaluation. That is where any change would show up first
What the research actually shows
- The gut effect is not disputed. Sorbitol is incompletely absorbed; malabsorbed polyols draw water into the bowel and are fermented, causing wind, bloating, cramping and loose stools in sensitive people. This is why the warnings exist
- IBS trigger, for some. A blinded FODMAP reintroduction trial in IBS patients (Clinical Gastroenterology and Hepatology, 2024) found sorbitol triggered symptoms in about 23% of participants — fewer than fructans or mannitol, but a substantial minority
- Tolerance is individual and can shift. A 2024 paper in Cell (UC Davis) reported that in mice, past antibiotic exposure combined with high fat intake produced long-lasting sorbitol intolerance by depleting Clostridia that normally break sorbitol down. This is mouse work — the authors said clinical studies are needed — but it is a plausible explanation for why some people react and others don't
- Children get more per kilo. Nothing about sorbitol is specific to children, but a small body eating the same sugar-free sweets reaches a symptomatic dose sooner. India's declaration singles this out
- Not a cancer or organ-toxicity story. There is no regulator finding and no established human evidence linking dietary sorbitol to cancer, and we won't imply one
- Sorbitol (E420) is a medium-risk flag in LabelCop
- The reason is digestive, not toxicological: sorbitol is only partly absorbed, the rest is fermented in the gut, and that causes wind, bloating and diarrhoea
- The supporting fact is a legal one — EU law (Regulation 1169/2011) requires a laxative warning on foods with more than 10% added polyols, and US and Indian law require their own versions
- It does not carry a high-risk flag, because no regulator has restricted it and none has found toxicological harm
- The product's score takes a penalty — and you see the flag, the reason and the sources. No black box
Frequently asked questions
Is sorbitol banned anywhere?
No. It's permitted in the EU, UK, US, India, Canada and Australia, and no major regulator has banned or restricted it. What several have done is legislate a warning: the EU requires a laxative-effect statement above 10% added polyols, and US law requires "Excess consumption may have a laxative effect" where foreseeable intake could reach 50 g a day.
Why is there a warning if there's no daily limit?
They answer different questions. JECFA's "ADI not specified" is a toxicology verdict — no number needed. The warning is about digestion. Sorbitol is only partly absorbed in the small intestine; the rest is fermented in the colon, drawing in water and producing gas. Regulators dealt with that by requiring a label statement rather than by setting a limit.
How much causes diarrhoea?
No regulator has published a safe dose, and tolerance varies widely. The two figures written into law are markers, not thresholds of safety: 50 g a day in the US, and more than 10% added polyols in the product in the EU and India. Plenty of people notice wind and bloating well below the US number, especially from a single large dose.
Is sorbitol bad for IBS?
It's the "P" in FODMAP and is excluded during a low-FODMAP elimination phase. A 2024 blinded reintroduction trial in IBS patients found sorbitol triggered symptoms in roughly 23% of them — a real trigger for a substantial minority, not for everyone. Work with a dietitian rather than self-diagnosing off a label.
Is sorbitol natural?
It occurs naturally in apples, pears, prunes and stone fruit. The additive is manufactured — the FDA's own rule describes it as made by hydrogenating glucose solutions. Either way your gut treats it the same, so "natural" isn't the useful question here.
Is sorbitol safe for diabetics?
It raises blood glucose far less than sugar, which is why sugar-free and diabetic ranges use it, and it isn't a toxicological concern. The catch is dose: those ranges tend to use a lot of it, so the digestive effects arrive faster. It still contains calories. Anyone managing diabetes should take dietary advice from their clinician, not from a front-of-pack claim.
