Is xylitol safe?
- EU: permitted; authorised dental health claim for 100% xylitol chewing gum; laxative-effect statement required above 10% added polyols
- WHO/JECFA: "ADI not specified" — no numerical limit, and that judgement dates from 1983
- US & India: permitted; the FDA treats it as generally recognised as safe

What is xylitol?
- A sugar alcohol (polyol) — about as sweet as sugar, with roughly 40% fewer calories
- Originally from birch and beech wood; now mostly produced by processing corn cobs and other plant material
- Barely raises blood glucose, which is why diabetic and low-carb products use it
- Your body makes it too — xylitol is a normal by-product of human metabolism, which matters a great deal when reading the heart research below
- Label names: xylitol, E967, INS 967 — or hidden inside "sweeteners" or "polyols" on the nutrition panel
Which foods contain it?
- Sugar-free chewing gum and mints — the classic use, and the one the EU health claim is about
- Sugar-free sweets, lozenges and throat pastilles
- Diabetic-friendly and "keto" ranges, protein bars, no-added-sugar biscuits
- No-added-sugar jams, marmalades and spreads
- Toothpaste, mouthwash and dental products (not food, but a real source of exposure)
- In India: most often in imported sugar-free gum and mints, diabetic-friendly ranges and keto products sold online and through quick-commerce — look for INS 967 in the ingredient list, because front-of-pack "sugar free" tells you nothing about which sweetener was used
What the regulators actually say

Why do regulators disagree?
- They mostly don't — and that's the problem. Everyone permits xylitol, largely on a judgement made in 1983
- The EU contradicts itself on the same pack: an authorised claim that xylitol gum is good for children's teeth, and a mandatory statement that too much of it is a laxative
- No numerical limit anywhere. "ADI not specified" means regulators saw no need for a number — it does not mean the amount doesn't matter
- The 2024 heart research arrived after every one of these decisions. No regulator has acted on it; EFSA's sweetener re-evaluation is the place it would surface first

What the heart research actually found
- 2024, European Heart Journal (Witkowski et al., Cleveland Clinic): in a validation group of 2,149 patients, those in the highest third of blood xylitol had about a 57% higher rate of heart attack, stroke or death over three years (adjusted hazard ratio 1.57, 95% CI 1.12–2.21)
- The mechanism arm: ten healthy volunteers drank 30 g of xylitol; platelet reactivity rose within 30 minutes. Laboratory and mouse work found xylitol enhanced platelet aggregation and shortened clotting time
- The caveat that matters most: the patients' blood was drawn after an overnight fast, and xylitol clears the blood in hours — so the levels measured were most likely made by the body, not eaten. Association with blood xylitol is not the same as association with dietary xylitol
- Other caveats: both cohorts were patients already undergoing cardiac assessment, diet was not recorded, and the human drink study had ten people
- Where it stands: unresolved and actively debated — the same journal has since published correspondence arguing both ways. Enough signal to flag; not enough for a regulator to have moved
- Xylitol (E967) is a medium-risk flag in LabelCop

Frequently asked questions
Is xylitol banned anywhere?
No. It's permitted in the EU, UK, US, India, Canada and Australia. In the EU it's allowed at quantum satis in listed categories, mostly energy-reduced or no-added-sugar foods, and packs with more than 10% added polyols must carry a laxative-effect statement.
Why does the EU allow a health claim and a warning on the same pack?
They answer different questions. Regulation 1024/2009 authorised the claim that 100% xylitol chewing gum reduces dental plaque, a caries risk factor in children, on condition of 2–3 g at least three times a day after meals. Regulation 1169/2011 separately requires the laxative statement above 10% added polyols. Teeth and gut, two rules.
Does xylitol cause heart attacks or strokes?
Not established. The 2024 European Heart Journal study found a 57% higher three-year event rate in the top third of blood xylitol, plus a short-term platelet effect after a 30 g drink. But blood was sampled fasting, so the xylitol measured was probably made by the body rather than eaten, and the cohorts were already high-risk cardiac patients. No regulator has changed xylitol's status; the authors asked for long-term studies.
How much is too much?
No regulator has set a number. The practical marker in EU and Indian law is the 10% added-polyols threshold that triggers the laxative warning. Single large doses are the ones most likely to cause wind, bloating and diarrhoea — tolerance varies a lot between people, and builds with regular use.
Is xylitol dangerous for dogs?
Yes. The FDA warns that xylitol is toxic to dogs, causing a sudden, sharp fall in blood sugar that can be fatal if untreated. Sugar-free gum, mints and peanut butter are the usual culprits. It's a species difference, not evidence about people — but keep these products away from pets.
How do I avoid it?
Look for xylitol, E967 or INS 967 in the ingredients list, and check the "polyols" line on the nutrition panel — front-of-pack "sugar free" tells you nothing about which sweetener was used. Or scan the label with LabelCop and it's flagged automatically, with the regulator reasoning shown.
